The Maritime and Coastguard Agency (MCA) has published the MSF5100 application form. It is for owners and operators of ships and fishing vessels to request a survey or inspection.
The form also contains information about health and safety standards and the arrangements you must make to make sure the surveyor can inspect the vessel safely.
